This policy describes how Dedicated Home Services Corp (“Dedicated Home Services,” “we”) handles information you send through the website intake form and ordinary email or phone contact. It is written for this static website at https://codhscorp.com.

What we collect
The intake form asks for first name, last name, phone, email, county, payer or program, and a message. A hidden field is used only to catch automated spam; people should leave it empty.
If you call or email us, we receive whatever you choose to tell us.
Do not send Social Security numbers, Medicaid IDs, diagnoses, or other medical records through the public form. Call (720) 591-0000 during office hours if that information is needed another way.
How we use it
We use this information to respond to intake questions and, if you become a client or applicant, to continue that conversation. Form submissions are emailed to info@codhscorp.com. We do not sell this information. We do not use the form to run advertising.
Where it is stored
The website is hosted on ordinary shared hosting. The form handler sends an email; it does not write your answers into a public file on the site. Email is then stored in our inbox and, if we open a record, in our operating systems. Server logs may include an IP address and browser type.
How long we keep it
Intake messages are kept as long as needed to reply and, if you become a client, as long as our records require. You can email info@codhscorp.com to ask us to correct contact information we have from the form.
Job applications
When we accept job applications, we use ClearCare (dedicatedhs.clearcareonline.com) as our applicant vendor. That process is separate from the website intake form. See the careers page for how we post openings.
What this site does not do
The website intake form does not embed a third-party form vendor. Job applications, when we are hiring, are submitted through ClearCare, not through the intake form. This site does not publish reviews. If we add analytics later, we will update this page.
Children
A parent, guardian, or case manager may contact us about a child’s services. Do not send a child’s full medical record through the public form.
Changes
Effective August 27, 2026. If this policy changes, we will update this page.
